Position: I/DD Care Management Extender (2 Positions)

Salary Range: $40,000 - $42,152

Position Description: The position will be part of a multidisciplinary care team that delivers integrated, whole-person care management to addressing member’s physical health, behavioral health (BH), intellectual/developmental disability (I/DD), traumatic brain injury (TBI), long-term services and supports (LTSS), and/or pharmacy needs in addition to unmet health-related resource needed.
Eastpointe’s Care Extenders support Care Managers in delivering Tailored Care Management and fulfilling member contact requirements.
The vision is Care Extenders will help advance Eastpointe to meet the needs of members, build efficient care teams by creating additional workforce capacity and allow Care Managers and Care Manager Supervisors focus on key tasks for assigned members as well as permit them additional time for members with intensive or complex needs.
This person will address critical community needs of individuals and work with members and/or family members/caregiver(s) to provide information and support throughout the process participating in Tailored Care Management.
Responsibilities:
Performing general outreach, engagement, and follow-up with members
Coordinating services/appointments (e.
g.
, appointments/wellness reminders, arranging transportation, etc.
)
Assist members in linking and advocating for appropriate services, in and out of traditional systems
Engaging in health promotion activities (as defined in Eastpointe’s Tailored Care Management Provider Manual) and knowledge sharing
Sharing information with the Care Manager and other members of the care team on the member’s past, current, and changing circumstances of importance as information arises and transitions occur.
Providing and tracking referrals
Providing information and assistance in obtaining and maintaining community-based resources and social support services
Participating in case conferences Support the Care Manager in assessing and addressing unmet health-related resource needs
Regular communication and coordination with Care Manager
Participate in all-mandated meetings for Tailored Care Management
Complete Tailored Care Management training curriculum
Education/Experience:
Must be at least 18 years of age.
High school diploma or GED with lived experience.
Be a parent or guardian of an individual with an I/DD diagnosis, TBI or a behavioral health condition.
Have at least two years of direct experience providing care for, and navigating, the Medicaid delivery system on behalf of that individual.
Be a person with lived experience with an I/DD or a TBI with demonstrated knowledge of, and direct personal experience navigating the North Carolina Medicaid delivery system.
