Description of Work "This position is designated Mandatory and Must report or stay over during adverse weather events.
" The Human Services Provider Program Coordinator II serves as the Home Manager and is responsible for the overall management program coordination and supervision of a certified Intermediate Care Facility/Intellectual Disabilities (ICF-ID) home including assigned shift personnel.
Job responsibilities include the following: Serves as Team Leader/Home Manager for assigned residents.
Ensures that the residents served are provided a comprehensive array of training, services and support that enable them to achieve maximum independence in self-care, activities of daily living; including gaining and maintaining significant relationships, self-management of ensuring the protection of their rights.
Designs programs within the home inclusive of writing objective plans.
Monitors program implementation.
Provides hands-on programming and training staff within the home.
Responsible for the supervision of all Residential Services staff assigned to the home.

Possess skills in individual treatment planning such as person-centered planning, individualized education planning and/or individual service planning and outcome documentation.
Demonstrated ability to coordinate professionals, paraprofessional, and direct support staff in the development, implementation and evaluation of person-centered services.
Demonstrated ability to manage and supervise the operation of a residential unit and to develop and maintain cooperative and cooperative working relationships with the residents, families and staff.
Ability to communicate effectively with professional and administrative personnel.
Physical Requirements: Must be able to lift a minimum of 50 pounds on a regular basis (6-8 times daily) Must be able to stoop, bend, walk, run short distances (50 yards) and climb stairs without limitations of a permanent nature Minimum Education and Experience Requirements Some state job postings say you can qualify by an “equivalent combination of education and experience”.

Bachelor's degree in a human services or related field from an appropriately accredited institution and two years of experience providing professional or direct services (either paid or volunteer) in a setting that serves individuals with disabilities.
